This position is responsible for screening patients and their families for healthcare needs and connecting them to related programs and resources.
Outreach Coordinators interact with patient and their family daily primarily over the telephone and through electronic communication.

Responsibilities:
The Outreach Coordinator is responsible for:
Initiating telephone contact for the purposes of completing a health risk assessment Connecting patients to the right program when needs are identified Escalating complex or urgent needs to a supervisor Management of electronic communication to send and receive survey results of a health risk assessment Documentation within the electronic medical record of assessment Developing patient?s/family self-management skills through education and resource provision Participating in orientation and continuing education of staff and students as appropriate (Frequently)
Qualifications:
Education:
High School diploma, with background in health care, public health, or related clinical field a plus.
Experience:
Excellent customer service skills with the ability to communicate effectively through multiple mechanisms including phone and writing.
Preferred experience working in healthcare in a patient facing role.

Estimated Salary: $20 to $28 per hour based on qualifications.

The check will bounce and the victim is left responsible.
