0 23 hours ago Full Job Description Position The Hereditary Gastrointestinal (GI) Cancer Business Care Coordinator is responsible for assisting in guidance and coordination of patient care for the Hereditary GI Cancer Program.
This position will work with a multi-disciplinary team to provide coordination of care for patients with inherited gastrointestinal cancer syndromes including but not limited to Lynch syndrome and Familial Adenomatous Polyposis.
The Hereditary Care Coordinator must be able to collect, organize, and summarize medical records both internally and from external sources in preparation for clinical appointments and procedures.
The position requires direct patient contact including obtaining consent for and entering information into the Hereditary Registry.
The position will also work closely with James Patient Access Services to schedule multiple coordinated appointments and follow-up with the patient through their health care journey.
The position will use software to track new and follow up patients through the care continuum.
Position Summary The Hereditary Gastrointestinal Cancer Business Care Coordinator will organize and coordinate the day-to-day operation of the Hereditary GI Cancer Program.
The Hereditary Care Coordinator functions as an integral part of the Colorectal Cancer Center and Cancer Genetics Program.
The Care Coordinator will use expertise to provide overall program management of the Hereditary program, provide clinical expertise on defining where patients are to be scheduled, collect, and interpret medical records, manage multiple data elements associated with the Clinic, and provide consultation and education on the Hereditary Program.
The Business Care Coordinator develops, aligns, and operationalizes the clinical, academic, and research activities to support the overall Cancer Program goals and to ensure the implementation of key OSUCCC-The James initiatives through interdisciplinary collaboration.
Duties and Responsibilities Duties 80% Hereditary Program Coordination Collects medical records, extracts data, and determines necessary medical appointments and procedures.
Coordinates scheduling of appointments, lab tests, and follow up calls or appointments.
Communicates efficiently and effectively with patients and team members.
Communicates with patients and staff members in a professional manner.
Facilitates timely scheduling of appointments, diagnostic testing, and procedures to expedite the plan of care and promotes continuity of care.
Liaisons with James Patient Access staff regarding scheduling management or concerns in the Hereditary Clinic and is affiliated caregivers.
Coordinates pretesting required appointments or procedures.
Organize and conduct in-services to share information about the hereditary program Read and accurately interpret clinical information per protocol and accurate data entry.
Monitor patients due for follow-up imaging and assist with scheduling scans and/or follow-up procedures Monitor and assist with follow-up of abnormal findings Monitor adherence to the operational and clinical protocols established for Hereditary GI Cancer syndromes Facilitate research participant screening with patients, RN staff and faculty.
20% Key Performance Operational Metrics - Data Management, Support, and Analysis Enter patient information into the chart and registrar (database) Adherence to recommended protocols developed for the Hereditary program Program compliance - yearly follow up, tracking of positive results, and other navigation compliance.
Term (Fixed Term) 40 First Shift.
Estimated Salary: $20 to $28 per hour based on qualifications.
