Social Worker (Home-Based Primary Care)

Incumbent independently conducts psychosocial assessments, evaluating the psychosocial functioning and needs of patients and their families to formulate and implement a treatment plan, identifying needs, strengths and coping skills in collaboration with the patient and family as appropriate. Incumbent provides case management to veterans within HBPC program, coordinating care and assuring that veterans receive needed benefits and services to increase functionality, independence, and quality of life. Incumbent attends HBPC team meetings and works closely with an interdisciplinary team to formulate a comprehensive care plan for HBPC patients. Incumbent establishes collaborative relationships with other team members and staff outside the department in order to more effectively coordinate patients' care. Incumbent assesses caregiver burden and makes plan to sensitively address caregiver burden. Incumbent establishes and maintains therapeutic relationships with veterans and families from diverse backgrounds. Incumbent is able to independently work with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills. Incumbent sensitively assists veteran and family in long term planning as disease processes progress. Incumbent has specialized knowledge in geriatric needs and disease processes in order to sensitively address needs. Incumbent has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology. Incumbent has knowledge of community and VA resources, and makes appropriate referrals to programs in order to best meet veterans' needs and coordinate veterans' care. Incumbent advocates on behalf of the veteran to ensure that services and benefits are obtained in a timely manner and in keeping with the VA's goals for excellence in customer service. Incumbent provides education and consultation to staff, students, patients, and families regarding programs and serves as a point of contact for the community for these programs. Incumbent remains current on VA policies and procedures relevant to program areas. Incumbent works with team to meet goals and performance measures within program. The incumbent effectively utilizes electronic resources required for providing services and completing reports. These include, but are not limited to, CPRS, VISTA, Microsoft Word, workload reporting, and program referral procedures. Incumbent documents work appropriately and in a timely manner in the medical record. Performs other related duties as assigned, including providing coverage to other Geriatric department social workers as needed. Work Schedule:

Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Social Worker, GS-11 (1) Experience and Licensure. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non- VA experience) and licensure or certification in a state at the independent practice level. OR (2) Education. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:
(a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. (b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. (c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. (d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. (e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. The position requires a valid government driver's license, knowledge of highway laws/safety, and familiarity with the geographic areas of jurisdiction. The incumbent must be able to operate a motor vehicle and perform local travel as needed. The incumbent may be asked to perform physical tasks requiring standing, bending, stooping, kneeling, climbing stairs, and lifting up to 25 lbs.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
